Interagency Readiness Solutions (IRS) is seeking a Non-Clinical Nurse Case Manager. The Non-Clinical Nurse Case Manager obtains individual cases from PHA, IBHS, self-referrals or command directed evaluations, at which time it becomes the case manager’s responsibility to assist individual Soldiers in becoming Fully Medically Ready for deployment. Case Manager assists with proactive help in obtaining and tracking necessary medical health appointments, medical documentation and maintaining communications as required by each individual Soldier’s specific situation. Case Manager works as an asset to G1 Medical as needed to ensure and/or return Soldiers to Medical Readiness for the NCNG.

Job Requirements

  • Maintain appropriate licensor and credentialing
  • Follow Soldiers from receipt of case until Soldier receives deployable status or documentation is sent for fit for duty evaluation, MEB or PEB
  • Supervise and delegate work to the Administrative Assistant (medical records tech)
  • Educate Soldiers on resources available to improve behavioral health readiness
  • Review medical records to determine medical readiness
  • Write and manage profiles
  • Consultations and communication with RNCO and Command
  • Document in MedChart and Health Tracker for every call, email and consultation
  • Draft letters and packets for NDPEB
  • Attend PHAs to screen Soldiers for case management and assist Medical Health Officers all over NC
  • Attend SRPs to assist unit in screening possible Soldiers that are non-deployable for behavioral health conditions
  • Educate Soldiers on their different options in regards to exiting the National Guard when needed
  • Daily, weekly and biweekly check-ins with Soldiers via telephone and/or e-mail -Routinely keeping RNCOs up to date on their Soldier’s case management status
  • Review medical documentation for possible LOD and FFD needs
  • Connect Soldiers with providers and resources as needed
  • Contact providers as needed for updates on Soldiers prognosis and coordination
  • Travel arrangements and reports for events
  • Coordinate appointments for Soldiers to meet with Medical Health personnel as needed
  • System access or experience in using IPERMS, MODS (e-profile, MHA, MEDPROS), SIBx, IDES, MEDCHART, HRR, eMMPS, AKO, GKO, BH Tracker and Sharepoint, multiple shared drives

Qualification Requirements

  • A current and valid RN license in North Carolina
  • Security Clearance or the ability to get and maintain a Security Clearance
